i once have the same problem, just that one period, 1/3 of my fishes die off one by one every 2-3 days, lost about 8 fishes in total. if your loach deaths have similar pattern like that, it could be internal parasite. it's unfortunate but it can still happen even if your tank is at its best.I have the patience of a goldfish....

also your water authority might change its chemistry from time to time without telling you. If that happen, given how big your weekly water change is, your fish might get shock from big change in chemistry. I would suggest to do many mini water change rather than an 80% water change at a time.I have the patience of a goldfish....

you can try to be on the safe side and treat the tank for internal parasite and/or fungal infection. My only suggestion is unless your tank is heavily overstocked, do smaller water change instead of 80% like that. One thing I remember before the time I had that problem with my tank I was doing someone similar to you, 50+% water change per week. Water chemistry difference can be dangerous if your water authority put in some new chemical into your water, which is why people suggest small frequent water changes over single large water change. Even so, do it every 3-4 days i guess. If your tank is well established big water change is not that necessary.I have the patience of a goldfish....
