Nitrates? Would be my first guess. Otherwise try adding some salt. Not certain what species it is. What are his parameters and what species? Maybe an Abei?

Tissue necrosis. typical of a species for brackish spending to much time without salt in the water. You might be able to save him if you gentle raise the salinity. 1.006 for young puffers up to 1.02 for adults. It has to be done gradually. (Might be to late for this poor fellow)
