Like holes in its head? I have seen oscars and flowerhorns like that. Hole in the head is usually caused by poor water quality. If thats the case you need to start doing some routine water changes. Pictures are not very clear but from your description that's what it sounds like.

Nothing on head. Both sides of body looks like scales are missing. Will try to get better pic. Water quality is fine, I change water weekly.

Nitrates are high. Between 80 an 160 looks like. I do have a good bioload in the tank but I have 3 canisters running that turn my 125 over more than 7 times an hour. I do not have carbon in the tank. I have sponge filters and polyfiber along with several biofilter levels. Change the water weekly. Just found a juvie bolivian ram dead as I'm typing this. Lots of live plants. Any suggestions on how to get this nitrate under control. Will do large water change tonight after work.

When was the last time you cleaned out your canister? If it's been a while you might want to clean them out. How large is your water change?

You just got some new fish. that will change the bio load. Big fish bring high bio load too. do a 90% water change and clean all the filters. Use the tank water to clean the filters to keep the bacteria seeded in the media.

Severums tend to be conspecific aggressive. It could be they are sorting out a pecking order. You can use melafix or aquarium (not reef) salt to help the wounds heal.
